Please read and understand all of the Statutes and Rules regarding fireworks before applying for a license. For assistance with finding fire department jurisdictions, please refer to the
Fire Department Lists and Information web page. One can search by town or county to obtain fire department contact information to verify areas of jurisdiction.
Each application must be printed and completed legibly; it does not have to be type written. You must either then mail or hand deliver the application in addition to the required fee and any the required documentation, to the address listed on the application.
Failure to complete the form in its entirety and sending all applicable required documentation may result in a delay or rejection of your license/certification. You will be notified by email regarding missing documentation so please provide a current email that you will receive this notification in a timely manner.
Each of the licenses, certifications or permits requires the application to be filed with the Division of Fire Safety at least thirty days before the start of activities for which the license is required.

This application is for a fireworks retail sales outlet. The license is valid from the time of issue to December 31st of the current year, unless earlier revoked or suspended. It must be renewed annually. In addition to the completed, signed application you must also supply the following documentation:
This application is for one sales outlet. The license is valid from the time of issue to September 1st of the following year or same year if you submit the application after December 31. It must be renewed annually and may be revoked or suspended prior to its expiration date. In addition to the completed, signed application you must also supply the following documentation:
This application is for Display Operators and includes lead shooters of outdoor displays. Please download all three forms below. Refer to the Procedures letter for complete instructions on applying for certification. In addition to the completed, signed application you must also supply the following documentation:
All applicants are required to have actively participated in at least five (5) performances and successfully pass a general knowledge examination. The license is valid from the time of issue and good for three (3) years. Renewal of the certification shall be granted to an operator without the need to retest, provided: (a) they are in good standing with the Division; and (b) they have actively participated in at least two firework displays during the past year.
This application is for Pyrotechnic Operators and includes lead shooters of stage, theater, film, indoor and close proximity. Please download all three forms below. Refer to the Procedures letter for complete instructions on applying for certification. . In addition to the completed, signed application you must also supply the following documentation:
All applicants are required to have actively participated in at least five (5) performances and successfully pass a general knowledge examination. The license is valid from the time of issue and good for three (3) years. Renewal of the certification shall be granted to an operator without the need to retest, provided: (a) they are in good standing with the Division; and (b) they have actively participated in at least two firework displays during the past year.
This application is for a permit to conduct a fireworks display and will only be issued to a certified fireworks operator and shall be valid for only one date and location (or alternate date). This permit is only if there is no permit required by the local jurisdiction for the event.
